I understand and agree that this release includes any claims based on the actions, omissions, or negligence of Abaco Youth Baseball, Abaco Strong, their employees, agents, volunteers, and representatives, whether an injury or illness occurs before, during, or after participation in any Abaco Youth Baseball program or activity. I also grant permission to Abaco Youth Baseball and Abaco Strong to use photographs of my child or myself for promotional purposes in digital or printed media.

Youth Sports Insurance is required. You may decline insurance and fee wilI be waived if your child has complete medical coverage. Please provide proof of insurance by uploading file below.
